Frequently Asked Questions
How soon after discharge should I have a DMR?
Ideally within 2-3 days of leaving hospital, but we can still help if it's been up to 2 weeks since discharge.
What should I bring to my appointment?
Please bring all your medications (including any from before hospital), your discharge summary, and a list of any questions or concerns.
What if I'm having problems with my new medicines?
Tell us immediately - we can often suggest solutions or liaise with your hospital team or GP to resolve issues quickly.
Is this the same as a Medicines Use Review?
No, DMR is specifically for patients after hospital discharge and focuses on preventing readmission through better medicine management.
